Terms of service
Last updated: 2026-10-27
These terms apply between you and TW Works (KvK 99463547) when you use FormHaven. FormHaven is a service for businesses; by creating an account you confirm you use it for your trade, business or profession.
The service
FormHaven gives each form you create its own address. We store the submissions sent to it, filter spam and email them to the address on your account, within the limits of your plan as shown on the pricing page.
Trial, plans and payment
- New accounts get a free trial of 14 days, no card needed. When it ends without a plan, your forms pause.
- Plans are paid monthly or yearly in advance through Stripe and renew automatically until you cancel. You can cancel at any time in Billing; your plan runs until the end of the period you paid for.
- Over your plan's monthly submissions, new submissions are saved but not emailed until you upgrade or the next month starts.
- We announce price changes at least 30 days in advance by email; they apply from your next renewal.
Your responsibilities
- Keep your login safe and your account email address working.
- Use FormHaven only for lawful forms. No phishing, no collecting passwords or payment card data, no special categories of personal data, no forms aimed at children, and no sending spam through the service.
- For the personal data your forms collect, you are the controller and our data processing agreement applies.
We may suspend a form or account that breaks these rules or endangers the service, and will tell you why.
Availability
We work to keep FormHaven available but do not guarantee uninterrupted service. Planned maintenance is kept short and announced where possible.
Liability
Our total liability per year is limited to the amount you paid us in the 12 months before the event. We are not liable for indirect damage such as lost profit or lost leads. This does not limit liability for intent or gross negligence.
Ending
You can delete your account at any time in Settings. Your forms and submissions are then deleted as described in the data processing agreement.
Law
Dutch law applies. Disputes go to the competent court in the Netherlands.
